 I once worked for a company (Kenan Systems) that wrote billing software for telcos, and I'm a big fan of Free Software and Open Source software. So I've been casually keeping a list of the open source billing packages that I know about.

ISPD - http://ispd.eburg.com/ - the first open source billing system that I know of. Largely written in C. Seems to have run out of steam, or perhaps (but unlikely) it does something useful so it's stable.
